
Reablement
Our reablement service aims to support service users to;

remain at home when they start to find things more difficult;

to recover after a fall, an acute illness or an operation;

avoid going into hospital unnecessarily;

to return home more quickly after a hospital stay;

to support service users in achieving their maximum potential for independence and to rebuild their confidence through motivation, focus on their strengths, and practical support.

Managing pain – It is hard to motivate someone if they are in pain. If the person has had an injury it may be very painful at first and it is important to work with the individual so they can do tasks at a tolerable level. The reablement team ensures that the service user has access to support from healthcare professionals so that pain is well managed.

Assessing risk and making sure that the environment is safe for people, e.g. facilitating the checking people’s kitchens and home layouts and the equipment they use.

Supporting in helping people to become more independent with the availability and use of relevant equipment

Working closely with other healthcare professionals, including physiotherapist, occupational therapist, speech and language therapists.
